Direct Pay Pricing

At our practice, we prioritize your health by offering a straightforward direct pay model. We do not accept insurance, allowing us to focus on providing you with personalized care without the hassle of insurance limitations. We accept most forms of payment including cash, check, Care Credit, Visa, MasterCard, and American Express. Payment in full is required at time of service. If you have any questions about our payment policy please contact our office.

• New Patient Appointment - $350

• Follow-up Appointments - $250

• Platelet Rich Fibrin Treatment - $900

• Microneedling / Placement Rich Fibrin Treatment - $1,000

• Intralesional Steroid Injections - $200

What You Should Bring

Please bring a photo ID, previous treatment or medical information, and recent lab results  to the appointment.

​Cancellation/No-Show Policy

If you must cancel your appointment, please give us at least 48 hours in advance notice. You will be considered a no-show if you miss an appointment and do not notify us in advance.  A no show fee or canceling within the 48 hour time frame will result in a fee of $175. For a cosmetic procedure such as PRF treatment and/or microneedling, the cancellation/no show fee would be $250 if made within the 48 hour time frame. We will make every effort to remind you of your appointment via email and/or phone, but please help us by always making sure your phone number and email address are updated during your visits. ​​

​​​Prescriptions

At the time of your visit you may be provided with prescriptions to take to your pharmacy to fill. Should you have any questions about your prescriptions please refer to the medication instruction sheet provided to you at the time of your visit or call our office for assistance.

​​​Prescription Refills

If you would like a prescription refill, please have the pharmacy fax us a request at 407-866-1181 and allow up to 3 business days for it to be processed.

​​​Lab Tests

It is sometimes necessary to order lab tests in order to obtain additional information about your condition and/or to monitor the effects of treatment. If lab work is required prior to a return visit to our office, we do ask that you complete it at least 48 hours in advance to allow ample time for the lab to process and send results to our office. It is your responsibility to ensure that the results of any lab testing are received by our office prior to your scheduled appointment. Results may be faxed to us or you may bring them with you.

​​​Medications

We ask that whenever you come in for an appointment that you bring a list of all current medications.

​​​Minors

A parent or the legal guardian must attend the initial appointment with the child and all appointments involving invasive treatment. Minor children must be accompanied by a parent or authorized individual 18 years of age or older for all appointments. In the event a parent or legal guardian cannot bring the child to a routine appointment, we need a specific form (the Parental Temporary Assignment of Authorization to Treat Minor Child) signed but the signature must be witnessed by a staff member of Jain Hair Loss Institute.​
